Instructions for Check Payments

  • We accept only checks drawn and payable on US banks with magnetic coding. We can not accept foreign currency or drafts which are not negotiable on a US bank.
  • Mail a letter listing the desired documents and delivery address along with your check payable to: MIDI Manufacturers Association, PO Box 3173, La Habra, CA 90632-3173 (USA).
  • Please include an email address (or phone number) in case we need to contact you about your order.

Instructions for Credit Card Payments

  • We accept only MasterCard, VISA, and Discover Cards only.
  • Send us a letter listing the desired documents, delivery address, your credit card number and card expiration date, card holder name and billing address (where you receive your credit card statement), and the card-holder's signature.
  • Fax your letter to (714) 736-9775. Or, mail your letter to: MIDI Manufacturers Association, PO Box 3173, La Habra, CA 90632-3173 (USA).
  • Please include an email address (or phone number, especially outside of USA) in case we need to contact you.
