It is certainly possible to do what you ask, although there may be quite a lot of work involved to complete the process, as no 'mechanical' conversion will be 100% perfect for you.

I've just done a search via Google using 'scan score to midi file' and there is a LOT of information there, and various different systems referred to, and I'm sure you'd get a lot of help there, far more that I could say.
